Do you need travel vaccinations?
Our Nurses can provide comprehensive travel health advice and arrange for appropriate immunisations. Please arrange an appointment for a travel consultation at least 8 weeks before departure if possible. Vaccines need time to take effect and some may require a course over several weeks.

Travel Assessment
We offer a full travel advice and immunisation service. If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 8 weeks) before your journey.
Please note that the nurse should get back to you 2-3 weeks after the form is submitted.

When you have completed it you can either email it to us at churchfields.surgery@nhs.net or you can print it off and hand it into reception. One of our nurses will then be in touch with you.

Travel Vaccination Prices
A charge will be made for certain immunisations and vaccinations which are not covered by the NHS. Anti-malarial medication will be prescribed on a private prescription.
Payment for non-NHS services must be made at the first travel appointment.
