Making a Suggestion

Your comments and suggestions are important to us, please click on the icon to the right to complete a form to submit any you may have. Please only use this form for comments about the practice and suggestions as to how we can improve our service to you.

Medical matters and official complaints cannot be dealt with by completing this form. If you have a query regarding a medical matter, please telephone reception on 01527 872 163 to make an appointment to see the appropriate person. Making a Complaint

We hope that we can sort most problems out easily and quickly, often at the time they arise and with the person concerned. However, if you wish to make a formal complaint, please do so as soon as possible, ideally within a matter of a few days. This will enable us to establish what has happened easily. If doing this is not possible, your complaint should be submitted: 

  • within 12 months of the incident that caused the complaint 
  • or within 12 months of the event coming to your notice.

Alternatively, we can provide you with a separate complaint form for you to register your complaint (you can download this here along with our complaints policy leaflet.) This also includes a third-party authority form to enable a complaint to be made by someone else.  

We will acknowledge your complaint within 3 working days and aim to have fully investigated it within 30 working days of the date it was received.  If we expect investigations to take longer than this, we will inform you and keep you updated as the investigation progresses.  

When the investigations are complete your complaint will be determined; you may then receive a formal reply in writing, or you may be invited to meet with the person(s) concerned to attempt to resolve the issue.
