patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we have set up a face to face patient representation group so that you can have your say. We aim to meet on a quarterly basis and ask questions, such as what you think about our opening times, the quality of care or service received.

You can email the PPG at churchfieldsppg@gmail.com

Please note this email address is checked by PPG members, and therefore should not be used for individual complaints or confidential medical information.

Approximately fifteen patients attend the group meeting from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

If you wish to express an interest to join the patient representation group, please click the link below to open the sign-up form and complete all the fields.
